Only member congregations can apply to the CUC on behalf of prospective lay chaplains. Please consult the Lay Chaplain Application Process guide for detailed instructions on how to register Lay Chaplains.

The CUC is required to notify vital statistics when lay chaplains move. Please email us the following information:
- Name
- Full address of both old and new residence
- Phone number (both old and new if applicable)
- Marriage license number
- If you are moving congregations, the name of both old and new congregation
